Takeda is committed to support patients with gastrointestinal conditions such as short bowel syndrome. Takeda initiated and funded the myPNtracker app to assist people who require parenteral nutrition support. myPNtracker allows these patients to track their weight and many aspects of their fluid/nutrient intake and output. Collecting this data may help patients take an active role in the management of their condition.

myPNtracker was developed in collaboration with short bowel syndrome experts. The intuitive interface makes it quick to enter changes in weight and track both daily input values—oral fluids, IV nutrition infusions, IV fluid hydration and salt capsule—and daily output values—urine, stool, stoma or vomit, daily wellness, fluid balance.

Healthcare professionals can register on myPNtracker.com to monitor a patient’s progress, if the patient decides to share their data with them (optional feature).

myPNtracker is available internationally for use in a variety of languages including English, French, German, Italian and Spanish.

Technical details:
Patients have complete control over whether or not data is shared, and with whom it is shared. For added security all data is encrypted and the app has an added passcode security layer on top of the security features of the mobile device.

The app also includes a reminder feature to track appointments with healthcare professionals, reminders to take medications and to make regular data entries.

If patients using the myPNtracker app decide to share their data, healthcare professionals (HCPs) can view their patient’s data from the myPNtracker.com website.

myPNtracker may allow you or your coordination nurse to see reports related to your patients’ nutritional health between visits, providing these patients have chosen to share their data with you (optional feature).
